SBA Needs Checklist
In order for us to consider your request for an SBA loan, we will need the following information and documents. Complete information will be necessary to process your application. Copy forms as needed. Please remember to sign and date all forms and financial statements.
Complete
- Loan Request Form
- History of Business and/or Business Plan – Explain benefits desired from loan.
- Schedule of Business Debt – Figures must balance to Interim Statement provided.
- For each owner, partner, 20% or more stockholder, corporate officer and guarantor, provide the following:
a.Personal Financial Statement (not older than 90 days) must be joint if married – SBA Form 413 (Exp 1/31/18).
b.Personal Tax Returns for 3 years – Signed and Dated
c.Resumes
d.Borrower Information Form – SBA Form 1919 (Exp 4/17/17)
e. Credit Check Authorization (separate sheets unless married and applying jointly)
f. Residential Appraisal Notice - Provide 2 years projections for all new business acquisitions OR existing businesses when historical data does not show ability to repay. The first year needs to be provided on a monthly basis.
- If you own an existing business(es), please provide –
a. Current Income Statement and Balance Sheet
b. Copies of Complete Tax Returns for past 3 years – Signed and Dated - If purchasing a business, please include –
a. Sales Contract outlining terms of the sale.
b. Copies of Complete Tax Returns for past 3 years – Signed and Dated by Seller - Completed IRS Form 4506-T (Vers 8/14) –
a. If Change of Ownership, on Seller.
b. If Existing Business, on Borrower Entity. - Source of Capital Injection – Attach Statements
